How would payers integrate into the FtP Program?

A: Payers can play multiple roles.  They may require participation in transformation as a requirement of contracting.  They may help pay for the transformation.  They may learn from the transformation efforts that can be aligned with regard to services contracted.  They may integrate their care management efforts with multiple pharmacy transformation domain efforts (such as communicating with other care team members or follow up care).  There are tens, and perhaps hundreds, of different ways for payers to get involved.  

Could separate payer programs with some similarities be lumped into one Practice Transformation Team application if the multiple parties were supportive?
